Strive, Aigilx and Tandigm Health announce partnership with Nextgate

 

Three health services organizations partner with healthcare enterprise identification company Nextgate.

NextGate, an enterprise-wide patient identification platform, recently announced partnerships with Aigilx Health, Inc., an interoperability and HIE services company, with Strive Health, which focuses on value-based kidney care and with population health services provider Tandigm Health.

Nextgate also announced partnerships with Strive Health, the national leader in value-based kidney care and Tandigm Health, a leading population health services organization.

Aigilx Health will integrate NextGate’s market-leading Enterprise Master Patient Index (EMPI) with its premier Meridian Data Aggregation Platform (DAP) to provide a comprehensive, fully-integrated view of clinical, community and lifestyle data for HIEs and CIEs (Community Information Exchanges).

The partnership between NextGate and Aigilx Health already supports Rochester RHIO, a regional HIE serving 14 counties and 1.5 million residents in New York State. It is the first HIE to utilize the Meridian DAPsolution.

Strive Health’s partnership with Nextgate plans to enhance Strive’s whole-patient solution for kidney disease populations with accurate, consistent patient identification.

Utilizing NextGate’s market-leading EMPIon Amazon Web Services (AWS) to unify and match the right data to the right individual, Strive’s clinical team of Kidney Heroes™ will have  access to comprehensive, reliable high-quality data and analysis of its thousands of chronic kidney disease (CKD) and end stage kidney disease (ESKD) patients.

Healthcare providers and payors rely on Strive Health’s comprehensive kidney care model of predictive analytics and high-touch care teams to slow disease progression, reduce hospitalizations, engage patients, and improve quality of life. The addition of cloud-based patient identification technology from NextGate will support Strive with a complete and accurate view of patient data across the continuum to effectively identify gaps in care, avoid acute events, and ensure the right interventions at the right time. The EMPI will also impact Strive’s ability to effectively track its 15,000 kidney patients across six states for population health analytics and quality reporting under value-based care arrangements.

Tandigm Health selected NextGate’s cloud-based EMPI and Provider Registry as strategic components for fully-connected, person-centric care. The implementation will create a sustainable network of accurate, comprehensive patient and provider data by uniquely identifying and matching individuals to their records for highly collaborative care and fluid information exchange.

Patient identification and cross system interoperability are critical aspects of healthcare delivery for Tandigm Health, which serves more than 100,000 patients across its growing network of 1,350+ affiliated physicians and specialists. Deployment of NextGate’s EMPI solution on Microsoft Azure will enhance positive patient identification at every touchpoint throughout the Tandigm Health physician network.
