Neuroglee Health Expands Dementia Care Collaboration with Mayo Clinic

BOSTON — August 12, 2025 — Neuroglee Health has expanded its partnership with Mayo Clinic to deliver comprehensive, at-home cognitive and dementia care, building on their joint work in developing Neuroglee Connect™. The program incorporates principles from Mayo Clinic’s HABIT Healthy Action to Benefit Independence & Thinking® initiative, aiming to preserve cognitive function and independence for people with mild cognitive impairment.

Under the expanded collaboration, patients will have access to longitudinal virtual dementia care provided by advanced practice nurses, licensed social workers, pharmacists, and care navigators, with oversight from neurologists and geriatric psychiatrists. The approach combines high-touch support with remote monitoring and focuses on proactive symptom management, timely testing, and care partner engagement.

“This expanded collaboration is a milestone in our mission to lower the total cost of dementia care while improving outcomes, access, and experience for families,” said Aniket Singh Rajput, Founder and CEO of Neuroglee Health. “As value-based care models grow, this partnership demonstrates that we can scale a personalized, technology-enabled care solution to meet both patient and provider goals.”

The initiative aligns with Neuroglee’s participation in the Center for Medicare & Medicaid Innovation’s GUIDE Model, a federal program designed to expand access to comprehensive dementia care for Medicare beneficiaries. GUIDE-eligible patients across Mayo Clinic and partner sites will be enrolled at no cost to patients or families.

The financial impact is significant: dementia-related care costs Medicare about $22,000 per patient annually—nearly three times that of patients without the condition. Neuroglee estimates its model could save more than $6,300 per patient each year through early detection, stage-based care adjustments, and stronger support for caregivers.

By combining clinical expertise, personalized technology, and a scalable delivery model, Neuroglee aims to improve quality of life for patients and caregivers while reducing costs for payers and health systems.
