Aledade ACOs Generate Over $1 Billion in Medicare Savings in 2024

BETHESDA, Md. – September 2, 2025 — Aledade, the nation’s largest network of independent primary care practices, announced that its accountable care organizations (ACOs) saved Medicare more than $1 billion in 2024 through the Medicare Shared Savings Program (MSSP). The savings were achieved by improving care quality and outcomes for over 1 million Medicare beneficiaries, underscoring the impact of value-based care models.

According to recently released CMS data, Aledade ACOs accounted for nearly 20% of total MSSP shared savings, while caring for about 11% of all MSSP beneficiaries. Aledade’s most experienced ACOs, representing a quarter of its beneficiaries, achieved savings exceeding 10%, compared to 5% for beneficiaries in non-Aledade ACOs.

In 2024, Aledade partners:

  • Prevented nearly 96,000 hospitalizations and 167,000 emergency visits
  • Conducted over 800,000 annual wellness visits
  • Scored 4% higher than the national average for hypertension control, improving chronic disease management

Community Health Centers Achieve Strong Results

Community Health Centers (CHCs) face unique challenges in delivering care to underserved populations. More than 25% of all CHCs in MSSP are part of Aledade’s network. Aledade’s CHC Center of Excellence provides tailored resources and training to help these organizations succeed in value-based care.

In 2024, Aledade’s 10 CHC-only ACOs achieved:

  • $585 in savings per beneficiary, a 38% year-over-year increase
  • $96 million in total shared savings, up 62% year over year

High-Performing ACOs Lead the Way

Aledade’s highest-performing group, the Louisiana ACO, achieved more than 17% in savings and ranked No. 1 nationally for timely appointments, care coordination, and provider communication.

By the close of 2024, Aledade was partnered with 1,900+ primary care organizations across 59 ACOs, collectively serving over 1 million Medicare beneficiaries.
