Advance Directives Partnering with a PHR Platform for First Time

Partnership with Get Real Health and ADVault, Inc., to enable secure creation and sharing of digital advance care plans

For the first time anywhere in the world, a personal health record platform will include a digital emergency, critical and advance care planning service. The groundbreaking move comes through a partnership between Rockville, Maryland-based Get Real Health, creator of the patient engagement platform InstantPHR® and the data aggregation and development platform CHBase™, and Dallas, Texas-based ADVault, Inc., developer of MyDirectives® and MyDirectives MOBILE™.

Making MyDirectives available through InstantPHR empowers healthcare consumers to easily and quickly share vital information with doctors about who can speak for them during a health crisis if they cannot communicate, in addition to their medical treatment goals, preferences and priorities. Having real-time access to a person’s MyDirectives plan enables medical teams to make better decisions within the context of patients’ directives. Consumers can record their thoughts about medical treatments, palliative and hospice care, organ donation and autopsy, and other critical personal information on the device and in the format that is most convenient and comfortable for them.
